End-of-life support services

The end-of-life experience affects us all in various ways. If you are diagnosed with an illness that will shorten your life, these support services will help you maintain a good quality of life.

For services not available locally, we've included state and national hotlines and website links.

End of life services generally fit into five categories:

includes medical practitioners, specialist services, geriatrician, pain and medical management, pharmacies, palliative care, dietician, podiatrists, naturopaths, masseurs, physiotherapists, health support and rehabilitation.

After Hours General Practitioner

Free phone service after hours. Calls are answered by a registered nurse who can advise if you need medical advice, need to speak to a local doctor, make an appointment for the following day, attend the hospital for immediate assessment or call an ambulance.

Ambulance NSW Authorised Care Plans

Under the Authorised Care Plan program, paramedics can provide care for patients with complex needs and at end of life without needing special instructions from patients’ doctors. Forms are available on the web site and must be filled in by your GP.

At-home health support

Most at-home services provide physical support including nursing, clinical care and palliative care, personal care such as showering, assistance getting dressed as well as services such as physiotherapy and podiatry. Check each provider to find out which services are included.

Moruya District Public Hospital

Acute care hospital supported by Visiting Medical Officer (VMO) specialists and VMO GPs providing general surgery, general medicine, obstetrics, paediatrics, oncology, renal dialysis, high dependency care, and emergency services. Specialists in multidisciplinary cancer care teams (MDTs); provides some cancer surgery, and the hospital provides treatment for people with cancer including chemotherapy and surgery in collaboration with the Eurobodalla Oncology Service Chemotherapy Unit.

Palliative Care Service – Eurobodalla Community Health Service

Specialist Palliative Care nurses provide assessment, advice, support and care planning on a consultative basis for patients at home, in acute care facilities and RACFs, working collaboratively with primary care providers. Monday-Friday specialist nursing.

Physiotherapy

Assistance with injury recovery or managing chronic health conditions such as diabetes, chronic and complex pain, post-surgical rehabilitation, oncology and cardiac rehabilitation.

South Coast NSW Voluntary Assisted Dying Support

Local support and case management for those seeking to access the voluntary assisted dying process.

NSW Voluntary Assisted Dying Care Navigator Service

Information and advice on voluntary assisted dying. Monday to Friday, 8.30am to 4.30pm

includes providing meals, mobility aids, special equipment, cleaning services, respite, transport, home visiting, gardening, home shopping, internet and technology support and visiting hairdressers.

At-home support services

For those with complex care needs, most services are funded under the My Aged Care Home Care Packages scheme. Most at-home services provide a range of support including nursing, domestic and personal assistance, shopping, social support, home and gardening maintenance, respite and transport.
